Eligibility requirements
You can become certified as eligible for
service in either of two ways:

or request one from VOTRAN by calling one of the Gold Service
phone numbers.
The Eligibility application must
be filled out completely
to be
considered for transportation services from Votran. The Eligibility
application will be
reviewed by Votran and a determination will be made based on your application, if you
qualify for VOTRAN GOLD SERVICES. The eligibility process can
take up to twenty-one (21) days to complete. If you are approved for Votran
GOLD SERVICE, the scheduling of future trips is a quick and easy process.

Persons can be sponsored by and certified as eligible by a social service agency and not
have to repeat the registration in the VOTRAN Gold office.
When transportation is
available
Greater Daytona Beach Area
6 a.m. – 6 p.m. Monday through Saturday
Southeast and West Volusia
6 a.m. 6 p.m. Monday through Saturday
When to telephone for
information
Votran Gold reservations may be made from 7 a.m.-4
p.m. Monday-Friday.
Cost of services
A $2.50 one-way fare is charged for Gold service.
Exact change is required. Votran drivers do not carry cash.
A certified personal care attendant assisting the passenger will not be charged. A
companion will be charged a $2.50 one-way fare.
Scheduling transportation
Qualified persons
or
their personal care attendants must request transportation at least one day in advance of
travel. Reservations can be made no more than 7 days in advance. When calling the Gold
reservations office, please do not forget to make reservations for personal care
attendants or companions desiring transportation. Please do not forget to advise the
reservation office of any special assistance services which need to be provided.
Subscription service
is available for customers who require transportation service to the same destination
at least three or more times a week. Only one telephone call is necessary to make a
subscription service reservation. However, the reservations office must be advised of any
change in scheduling needs.
Medicaid medical
transportation service (non-emergency) is coordinated and scheduled by the
Votran reservation office for persons qualified for transportation under the Florida
Medicaid program. Please have your Medicaid number handy and exact information on location
of pickup and destination. This non-emergency transportation is provided by Votran and
private operators in Volusia County. A co-payment of $1 is requested.
